6th Annual Healthy Day! 5K Walk.Run & Health Fair

All UFCW Local 770 members with a PPO plan can earn up to $150.00 for your Health Risk Account by participating in a 5K (or longer)! Fill out  the form at our HRA table and we will submit it to the Trust Fund on your behalf or you can mail it in yourself.  This is a qualifying 5K event and many members in the last five years have enjoyed the ease of putting up to $150.00 in their Health Risk Accounts while having fun!  Jump start your "Healthy Day" with our 5K event.

 

You can also add $ to your account by filling out the Health Risk Questionare and Phyician's form provided on race day at the HRA table. So easy!
